now is staying healthy and living a full life. You might be looking for a
support network of other survivors to share your experience or perhaps you
would like to support others who are currently going through cancer treatment.

Due to the growing number of cancer survivors, The Cancer
Center has enhanced survivorship services to patients and their families to
help them understand and learn to live with cancer. To meet these needs the
following services are provided:
§ Survivorship care plans are provided
by Jeanne Oelerich, nurse practitioner, within one month of treatment
completion. This plan includes a treatment summary, list of late and long term
side effects of treatment, recommended surveillance for recurrence, and
resources for survivorship.
A copy of this care plan is provided to the patient
and his or her primary physician.
§ Networking and support groups that
meet on a regular basis.
§ Using a psychosocial distress
assessment tool by patient care staff to screen for concerns and issues from
the start of treatment through survivorship
